For your convienience, the Healthcare Power of Attorney and the Healthcare Directive
(Living Will) have been combined together to form the Healthcare Package.
In some States, a health-care instructions directive (sometimes called a “living will”)
and a power of attorney for health care are two distinct documents. In other States,
the law provides that these kinds of documents may be combined in one writing. In many
of those States, the statutory form is so combined. So you might get one document that
covers both purposes: stating your health-care instructions and naming your health-care agent.

Power of Attorney for Heathcare
A power of attorney for healthcare, or a healthcare proxy, empowers someone
you trust (your agent) to ensure that your health care wishes are fulfilled.
This person should have, or be aware of the location of, your health care directive
and the wishes you have stated within that document. Additionally, should a
situation arise that is not covered within your health care directive, your
agent will be able to communicate your wishes rather than the decision being
left to the doctors or your entire family.

Health Care Directive or Living Will
A health care directive, or living will, allows a person to state their personal
wishes about life-prolonging medical treatment should they not be able to communicate
those wishes in person. These documents are a way for you to tell the doctors
how you want to be cared for. This can oftentimes save your family from having
to make difficult decisions during a traumatic time.
